Ages 2.5 to 15.5 

Participant Intake Form (Special Needs, Disability) - Only complete this form if your child has a special need or disability.*

Medication Form - Only complete this form if your child needs to take medication while at camp (including emergency medication, i.e. EpiPen).*

Refund / Cancellation Policy
All cancellation or transfer requests must be made with at least 5 business days notice prior to start date of activity. A $10 cancellation fee per activity per person will be deducted from the refund. Cancellation requests with less than 5 business days notice will not receive a refund.
